To obtain services, a client account must be established by completing and submitting a Prevention Center application. For more information about the application process, see Request Prevention Center Services.
Once a client account has been established with the Prevention Center, study participants may be scheduled for services.
If a study requires subsequent appointments for participants, the receptionist at the Prevention Center can make those appointments as each participant leaves the Center.
To schedule an appointment in the Exercise Facility, contact Matt Van Doren at mvandore@fhcrc.org or (206) 667-7915.
Study participants may have questions and will need information about how to get to the Prevention Center. Driving and parking directions to the Public Health Sciences building are available online.
Print copies of the directions are available also or participants can be given the links above to download a copy.Participants get free parking on the D level in the PHS parking garage. Parking validation is available by taking the parking stub to the receptionist in the Prevention Center or to the Front Desk receptionist on the first floor. Participants or study clients should see one of the clinic staff for validations required after 3pm, or participants will need to go to the first floor reception.
